REQUIREMENTS FOR ADMISSION

PRE-SCHOOL

Child must be 3 or 4 by November 30th and toilet trained
School needs a complete Immunization Record verified by your physician
Child must have a physical examination prior to entering school in September
School needs a copy of the child's Birth Certificate
School needs a copy of the child's Baptismal Certificate

KINDERGARTEN THROUGH EIGHTH GRADES

Child entering Kindergarten must be 5 years of age by November 30th
School needs a complete Immunization Record verified by your physician
Child must have a physical examination prior to entering school in September
School needs a copy of the child’s Birth Certificate
School needs a copy of the child’s Baptismal Certificate

 

Other Requirements (Kindergarten to Eighth)

Pre-admission testing required

Copy of last report card

Standardized testing results

 

 

Immunization Record (should include)

Diphtheria, Pertussis and Tetanus vaccine (DPT series) - 4 doses with 1 dose given on or after the fourth birthday, or any total of 5 doses.

Poliomyelitis vaccine - 3 doses of oral poliovirus vaccine (OPV) or enhanced inactivated polio vaccine (IPV), with 1 dose given on or after the fourth birthday, or any 4 doses spaced by a minimum of 1 month.

Rubeola (Measles) vaccine - 2 doses given on or after the first birthday separated by at least a month, or laboratory documentation of immunity.

Rubella (German Measles) vaccine - 1 dose given on or after the first birthdaym or laboratory documentation of immunity.

Mumps vaccine - 1 dose given on or after the first birthday, or laboratory documentation of immunity.

Hepatitis B vaccine - 3 doses or laboratory documentation of immunity.

Varicella vaccine - one dose of varicella vaccine on or after the first birthday or proof of disease or immunity will be required for children born on or after January 1, 1998.

Pneumococcal Conjugate vaccine (required for PreK) - series and an annual influenza vaccine.

Meningococcal Conjugate vaccine (entering 6th grade) - one dose and one dose of Tdap.

A physical exam must be completed on or about the fifth birthday or within one year of starting schoold and signed by the physican on the form provided by the school.

 

Please note: State law will not permit us to admit any child whose immunizations have not been completed and documented, specifically indicating a day, month and year.
